Your appointment at Chicago Consulting Physicians was scheduled by the state as part of the Social Security Disability determination process. Even if you already have a Primary Care Provider or specialist, the state may request a one-time medical or psychological exam to help gather the information needed to review your claim.
This visit is not a treatment appointment and does not replace care from your regular providers. We do not provide ongoing medical care, prescribe medications, or manage medical conditions. The results of your exam are sent directly to the state.

Yes. All scheduled appointments must be kept. If you already receive benefits, this appointment is part of a routine review or update requested by the state. The purpose is to ensure your case information is up to date.
This is a standard part of the disability process and does not mean there is a problem with your case.

If you are scheduled for multiple exams, you must keep ALL appointments
Please bring in an ID to check in with the Front Desk as well as your list of medications (if applicable).
If you are not fluent in English, please bring an interpreter
If you wear hearing aids, wear them or bring them to the examination

Most appointment will be about 1-2 hours long, Ophthalmology appointments will last 3-4 hours.

Our office is on the corner of Michigan and Lake street. In between the Citi bank and Five guys.
We are easily accessible by most public transportation systems. The closest CTA train station is located at Washington and Wabash, served by the Pink, Orange, Green, Brown, and Purple Lines
For parking options, we recommend using SpotHero or iParkit app. They provide a variety of nearby parking choices. Our office does not validate parking.

You will be prompted to enter an extension which can be found in the bottom left hand corner of any official letters the bureau has sent you. If you don’t have an extension wait to speak with an operator and they will connect you. OUR OFFICE IS UNABLE TO MAKE ANY CHANGES TO THESE APPOINTMENTS.

Once all appointments are completed, the state will mail you the decision. This usually takes about 4-6 weeks. If you need to check the status of your case, please call the Bureau of Disability at (800)225-3607. Thank you
